A meeting can look aligned because the record captures only explicit agreement. The review packet asks whether silence, hesitation, unresolved risk, or minority judgment had a route into the decision before consensus is treated as evidence.
Decision Route
AI meeting notes, remote collaboration tools, OKR dashboards, and management summaries can increase documentation while reducing independent correction. The evidence route is the path by which a dissent signal forces a counterexample review, owner update, or scope change.
Review
Leadership and governance claims become stronger when the dissent signal is named, linked to the meeting record, and allowed to update the decision boundary. Without that route, a team can convert silence into false consensus and optimize the wrong objective.
